Post-Secondary Transition Information

All students at Pathfinder have access to a wealth of Career Planning and PostSecondary Education Resources for planning their future.  One such resource is Pathful Connect where teachers in any course can facilitate activities aligned with a particular career field.  We have a full time Career-Tech Education Supervisor as well as Career Based intervention and Career Technology teachers on staff to guide students on their instructional and career readiness pathways.  Age Appropriate Transition Assessments are regularly conducted by Intervention Specialists to monitor students’ progress along their career readiness pathways.  Individual Transition goal progress, where applicable*, is reported quarterly.

In Ohio, students with disabilities ages 14 or younger, if appropriate, formally enter into the secondary transition planning process. A truly successful transition process is the result of comprehensive planning that is driven by the student’s preferences, interests, needs, and strengths. This comprehensive planning moves students toward successful future outcomes in competitive, integrated employment, education, and independent living.
