All students at Pathfinder have access to a wealth of career planning and postsecondary education resources to help them plan their future. One such resource is Pathful Connect, which allows teachers in any course to facilitate activities aligned with a particular career field. We have a full-time Career-Tech Education Supervisor, Career Based intervention, and Career Technology teachers on staff to guide students on their instructional and career readiness pathways. Intervention Specialists regularly conduct Age Appropriate Transition Assessments to monitor students’ progress along their career readiness pathways. Individual Transition goal progress, where applicable*, is reported quarterly.
In Ohio, students with disabilities ages 14 or younger, if appropriate, formally enter into the secondary transition planning process. A successful transition process results from comprehensive planning driven by the student’s preferences, interests, needs, and strengths. This extensive planning moves students toward successful future outcomes in competitive, integrated employment, education, and independent living.
